Certified individualized services include nursing, physical therapy, occupational therapy, speech pathology, medical social work, and home health aides.
Provides professional occupational therapy diagnostic and therapeutic services by collaborating with physician and interdisciplinary team members to develop and manage the patient’s plan of care.
Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression.
Observe/Assess the progress of the patient & prepare detailed progress reports.Collaborates with all disciplines to provide best patient centered treatment and care.
Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient, family and other caregivers
Develops comprehensive goals for patient treatment to maximize patient outcomes.Instructs and advises clients, family and other health team personnel including, when appropriate, home health aides, in the phases of occupational therapy in which they may assist the patient.
Provides documentation according to policy.Ensure compliance with relevant laws, regulations, or standards.Other duties as assigned.
Bachelor’s degree required; Master’s degree preferred.
Graduate of an accredited Occupational Therapy Program.
Minimum of one year of experience post-license in occupational therapy.
Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population, and to a lesser degree the pediatric and other adult population.
Demonstrated knowledge of the principles of growth and development over the life span.
Able to assess data reflecting the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs.
Valid New York State license as an Occupational Therapist.
KNOWLEDGE AND SKILLS·
